Yo-kai Watch 3's unique draw is the complete change in location. Unlike previous games, the protagonist's gender can not be chosen. The player switches between playing as Keita, who has travelled to the American town St. Peanutsberg, and Inaho, who remains in Japon. Since Yo-kai's origins are heavily rooted in local culture, St. Peanutsberg features a new set of Yo-kai called Murican Yo-kai. These Yo-kai are mostly based on American culture, such as historical figures and popular culture. Each character encounters different Yo-kai, explores entirely different locations, and has a different kind of Yo-kai Watch. While Inaho wears the Yo-kai Watch U, which has been previously featured in the anime, Keita wears the all new Dream Watch. Different Yo-kai summoning devices are also featured, such as the gun-shaped Yo-kai Blaster.

In addition to the fresh locations, the battle system is also completely new. While the classic mechanics such as purification, real time attacks, and Soultimate Moves are still in place, Yo-kai are no longer confined to a circle with six positions. Instead, they are positioned on a 3x3 grid and can be moved at any time. Positioning Yo-kai allows them to dodge attacks, give each other stat boosts, and gain other bonuses. Competitive battling is once again supported, and players gain an additional degree of customizing with the new Build-A-Nyan feature, which allows players to customize the appearance, skills, and stats of a Jibanyan variant.

Yo-kai Watch 3 contains over 600 Yo-kai, including a new tribe and a few Yo-kai that have been designed by fans. Although they can not be befriended, zombie Yo-kai also make an appearance in the new Zombie Time, which replaces Terror Time in St. Peanutsberg. Zombies will occasionally come out and chase after Keita, who must find a bell and ring it to ward off the zombies.
