The first Yo-kai Watch game begins with the protagonist obtaining their Yo-kai Watch in a scene that mirrors the anime; However, this is where the similarities end. The game's story follows the protagonist as they encounter various Yo-kai that are causing trouble around town and their attempts to help the people affected. While there is a linear story, the player is allowed to freely explore any areas they have unlocked to discover and complete over 100 quests. These quests usually involve battling Yo-kai that are causing trouble, summoning a specific Yo-kai, or fetching items.

With the opening of new quest lines and a huge dungeon after the main story is completed, players looking for a lengthy postgame will not be disappointed. Unlike future releases, Yo-kai Watch does not allow players to trade with each others or battle online, but local multiplayer battles are supported. Yo-kai Watch medals are supported through the use of the 3DS' camera to scan the QR codes on the back of them. By scanning a medal, a coin is given that can be used with the Crank-A-Kai to obtain a random Yo-kai. There is a total of 223 Yo-kai in Yo-kai Watch's medallium.
